The Jefferson City Police Department issued an Endangered Silver Advisory for a missing man Friday afternoon.
Jack J. Hazen is a 66-year-old white man with brown eyes. He is 5 feet, 8 inches tall and weighs 165 pounds, according to a Missouri Highway Patrol news release.
Hazen walked away after being discharged from Capital Region Medical Center on May 26, according to the news release.
Hazen has diagnosed probable dementia, alcohol addiction problems and a traumatic brain injury.
